Ward Round Participation

Department staff and students actively participate in ward rounds along with the medical staff in the ICU, Male Medical Ward, Female Medical Ward, Pediatrics Ward, OBG, Orthopaedic, and Surgical Ward. During the ward rounds the case sheets of the patients are being reviewed for assuring patient-specific drug therapy. Along with this patient medication interviews and wherever necessary proper interventions and recommendations are being made.

Patient Counselling and Education

Centre for Patient Counselling and Education (CPCE) was initiated to provide patient counseling services for chronic diseases like diabetes mellitus, hypertension, asthma, and chronic obstructive pulmonary disease are given to the outpatients. Apart from oral counselling, printed patient information leaflets (PILs) regarding diet, life lifestyle management are also being distributed to them. Pharmacists educate patients about potential drug interactions and provide guidance on how to take their medications to minimize risks. Patients are encouraged to inform their healthcare providers of all the medications they are taking. During counseling, the pharmacist should assess the patient’s understanding about his or her illness and the treatment, and provide individualized advice and information which will assist the patient in taking their medications in the safest and most effective manner.

Adverse Drug Reaction (ADR) Monitoring

Monitoring of adverse drug reactions is also carried out in the hospital where the staff and students actively monitor adverse drug reactions and report it to the peripheral centres. Monitoring patient who are at greater risk of developing ADRs. Monitoring patients who are prescribed drugs likely to cause ADRs. Assessing the patient's previous allergic status, patient's drug therapy, and possible drug interactions.

Medication Reconciliation

Pharm D students play a crucial role in medication reconciliation during transitions of care, such as hospital admissions and discharges. They ensure that a patient's medication list is accurate, helping to prevent potential interactions during these critical periods.

Treatment Chart Review

The patient’s medical record should be reviewed by the student Pharmacist in conjugation with the medication administration record. Recent consultations, treatment plans, and daily progress should be taken into account when determining the appropriateness of current medication orders and planning each patient’s care. All current and recent medicate on orders should be reviewed.

APPE PharmD Students will be trained on the following to excel their skills:

• Obtaining the most complete and accurate list possible of the patient’s current (and regularly) taken medications, known as the Best Possible Medication History (BPMH).
• Using the BPMH, the reconciliation is performed when writing medication order during admission, transfer and discharge
• Identifying and bringing any discrepancies to the attention of the team and, if appropriate, making changes to the orders and documenting all changes.
