Oral Myiasis by Chrysomya Bezziana Bizarre Sequel of Self Inflicting Injury - Dr. Indira M D

Oral trauma is most prevalent among special care children. Among these self-inflicting injuries are more common. One of the complications of self inflicting injuries In special children is Oral Myiasis. Oral Myiasis is an infected condition caused by dipterous larvae that feed on the host dead or living tissues, liquid body substances or ingested food. A 11-year-old spastic female patient reported to the Department of Pediatric and Preventive Dentistry with a complaint of pain and swelling in the anterior palate, since 5-6 days. Patient gave history of self-inflicted trauma with a wooden stick in the anterior palatal region followed by bleeding and pain. Clinical oral examination revealed, an oval, ulcerated lesion measuring 2cm X 1cm in 11 and 12. After the administration of local anesthesia, a cotton plug impregnated with eugenol was placed at the opening of wound and 14 live maggots were removed and sent for entomological examination. The patient was administered a single dose oral Ivermectin 3 mg and Amoxicillin 250 mg for 5 days. After 10 days follow up the wound appeared healthy and the healing was uneventful. The uniqueness of the case was in mentally challenged patient following self-inflicting trauma. These cases demand invasive procedures to be done under general anesthesia. The present case describes a non-invasive procedure done at chair side with the medications available in the dental clinic. Considering the socio-economic conditions this method of management was clinically effective and affordable.
